  • 2020’s

     

    2021
    • Partnered with several community hospitals (Scarborough Health Network, North York General Hospital and Markham Stouffville Hospital) and York Region Public Health for delivery of both stationary (at Carefirst One-Stop Multi-Services Centre) and pop- up vaccination clinics in Scarborough, North York and Markham with Carefirst’s medical health unit (MHU).
    • Awarded new service contract of Integrated Dialysis Care (IDC) from the Unity Health Network (St. Michael’s Hospital, St. Joseph Health Centre, and Providence Health Centre).
    • Soft launch of Carefirst Campus of Care Capital Project.

    2020
    • Surfing the waves of the COVID-19 pandemic: Wave 1: Provided Community Emergency Program for the vulnerable individuals;
    • Waves 2, 3: Appointed by Central Ontario Health as lead agency to work with 24 local service providers for “High Priority Communities Program” (HPCP) for south east Markham (a hot zone hardest-hit by COVID-19).
    • Approved by Ministry of Long-Term Care to develop Carefirst Campus of Care with a 120-bed long-term care home, a medical centre, a social community hub and a gymnasium in Richmond Hill.

    2019

    		 The Connecting Care Act, 2019 lays out the foundation to transform and strengthen the public health care system, e.g. through the establishment of Ontario Health Teams (OHTs). Carefirst became core members in several OHTs: Eastern York Region North Durham OHT, North York Toronto Health Partners, Scarborough OHT, and West York Region OHT in 2020

    2018
    • Partnered with Alzheimer Society of Durham Region and Seniors Care Network to set up a Memory Clinic in Carefirst One-Stop Multi-Service Centre.
    • Partnership with Ontario Shores to develop a psychotherapy clinic in Carefirst One-Stop Multi-Service Centre.

    2017
    • Achieved Accreditation Exemplary Standard from Accreditation Canada
    • Pioneered the development of a Medical Health Unit (MHU)
    • Partnered with Scarborough Health Network on the new Integrated Care for assisted Peritoneal Dialysis Patients at home

    2016
    • 獲得歐洲衛生護理設計大會健康護理及安康獎項為耆暉一站式多服務中心的創新建築設計。
    • 安省醫務衛生廳傑出護理服務質素及安全獎項之優異獎。
    • 安省社區服務支援協會 – 給予耆暉會首席行政總監梁樂欣女士卓越領袖獎。

    2015

    		 完成資金項目建設，並從耆暉一站式多服務中心運營。

    2014

    		 在士嘉堡開了另一個成人日計劃，共有3個成人日計劃中心（2個在士嘉堡，1個在列治文山）。

    2013

    		 獲得基建安大略省1200萬美元的貸款，用於建設耆暉一站式多服務中心。 為明天中心的建設舉辦地面開幕儀式。

    2012

    		 美國舊金山美國洛杉磯美術學習小組，以及耆暉的On Lok PACE護理模式。

    2011

    		 從安大略省的Trillium基金會 – 社區資本基金收到了50萬美元，用於在士嘉堡建造一站式多服務中心。

    2010

    		 培訓，學院和大學部批准執行耆暉職業培訓中心的運作。

    2009

    		 獲得長期護理部2009年創新項目的健康創新入圍獎：慢性腎臟早期識別和外展計劃，與斯卡伯勒區域腎透析計劃聯合發起。

    2007

    		 獲安省衛生廳撥款於烈治文山市成立「耆暉家庭醫療中心」，為區內提供基本醫療 服務。 再次獲加拿大衛生及社會服務優質鑑證局頒發2007至2010年的三年優質服務認證資格。

    2006

    		 獲烈治文山市政府批準撥款一百萬於烈治文山市建設「一站式多元服務中心」。 獲安省衛生廳撥款於烈治文山市及士嘉堡市成立「家庭醫療中心」，為華裔及亞裔社區提供更多元化醫療服務。

    2005

    		 獲烈治文山市政府批準撥款一百萬於烈治文山市建設「一站式多元服務中心」。 獲安省衛生廳撥成立全安省唯一的腎病支援中心。

    2004

    		 獲加拿大衛生及社會服務優質鑑證局頒發 2004 至 2007 年的三年優質服務認證資格;亦於同年成立「保護長者華語熱線服務」。

    2002

    		 伸展服務至密西沙加市，並成為皮爾區「約克區公益金」會員機構；同年開始提供華人安懷服務。

    2001

    		 創立「腹膜透析日間中心」為腎病病人提供成人日間中心活動。

    2000

    		 舉辦機構命名活動，正式由「華人耆老服務協會」 改名為「耆暉會」。

  • 1990’s

     

    1999

    		 在烈治文山設立約克區社區服務中心和成人中心。獲准成為「約克區公益金」會員機構。

    1997

    		 在Tam O’Shanter老年公寓開設了第二個支援住房，並在士嘉堡開了一個成人日服務。

    1995

    		 在多倫多的奧古斯塔大街91號的亞歷山德拉公園老年公寓開始了第一個配套住房服務。

    1992

    		 重新命名為耆暉會，以反映不斷擴大的服務範圍和地理覆蓋面。

  • 1970’s and 1980’s

     

    1989

    		 開了士嘉堡分行辦事處，為士嘉堡市服務。

    1986

    		 被批准為大多倫多聯合之路（現稱多倫多聯合之路）的成員; 通過新機構名稱多倫多中國老人家庭支援服務協會。

    1985

    		 成立為非營利慈善組織。

    1980

    		 更改為中國老人家庭支援服務協會，服務從單餐輪椅服務增加到一系列家庭支持服務。

    1976

    		 在多倫多唐人街運營中國式的輪椅式服務，營業成本為1000美元。 1978年，我們成立了中國餐飲與互助協會。
