Carefirst’s Home Care Team offers Home Care Services to support clients to continue to live a quality and independent life in the community.

Personal Care: Personal hygiene care such as grooming, bathing, dressing and toileting

Homemaking: Bathroom/kitchen cleaning, floor cleaning, vacuuming, laundry, grocery shopping and meal preparation

Caregiver Relief: Providing relief for family caregivers to enable them take a break from their caring role, from a few hours a day to around-the-clock care in home/care facility/hospital setting.

Service Highlights

  • Home Care (HC) Services – Funded by the Home and Community Care branch of the Local Health Integration Network (LHIN) (previously the Community Care Access Centre (CCAC) as a subcontracted service. Through the long-standing partnership with VHA Home Health Care, Carefirst provides personal support workers (PSWs) in the Central East, Central, and Toronto Central LHINs to assist clients with personal care and homemaking to maintain or improve their quality of life. Client Service Coordinators collaborate with clients and caregivers on an individualized care plan to help the client identify their goals and then develop a roadmap and leverage all available community resources in order for clients/caregivers to achieve their goals.  Home care services are delivered by PSWs under the supervision of nurses and supported by Client Service Coordinators (CSCs).  Carefirst also offers personal support, home making and cleaning services to clients on a private pay basis.
  • On-call workers: Since providing care to clients is a 24/7 endeavor, Carefirst now has on-call staff that field calls from clients, caregivers, service providers and workers outside of business hours. The on-call staff has allowed for a better work-life balance for Client Service Coordinators. In reducing the burden on CSCs, we are able to provide a better experience for both our clients and our staff.
  • 24-hr Hot Line Service: The hot line is set up especially for clients and caregivers so that they have access to Home Care 24 hours a day if necessary.
  • The Triangle Effect: Home care has improved its structure to promote ease of communication between the client/caregiver, their CSCs, and PSWs. This team-based approach to care will allow for clients/caregivers to communicate their needs in a more coordinated fashion so their team can respond in a more timely manner.
  • Major restructuring: In collaboration with Human Resources, Finance, Assisted Living, Home Care and Administrative departments, the Home Care team has and will continue to restructure in order to achieve the Strategic Imperatives outlined in the Strategic Plan. We will continue to explore and forge new partnerships in order to enhance our social impact and enhance our capacity. We will continue to diversify our workforce in order to better meet the needs of the diverse clients we serve. We will also continue to ensure that we are providing our staff with the best possible work environment because we understand that they are the foundation of our competitive advantage.
