Adult Day Program Picture 1The Adult Day Program provides structured and supervised activities for frail or socially isolated older adults. Our goal is to enable seniors to achieve and maintain their maximum level of functioning, to prevent premature and inappropriate institutionalization, and to provide respite and support for caregivers.

  • Seniors aged 55 or above and or those with physical disability
  • Seniors who experience functional limitations due to physical behavioural and cognitive impairments.
  • Seniors who experience social isolation with limited family and/or peer support
  • Seniors who are unable to participate independently in regular community program

  • Monday to Friday – 9:30am to 3:00pm
  • Saturday – 9:00am to 3:00pm
  • Extended Hours – 8:00am to 9:00am and 3:00pm to 6:00pm

Program and Activities

  • Health monitoring for individual participants
  • Daily light exercise
  • Social and recreational activities (craft, games, music, coordination and stimulation activities, group discussions outings, etc.)
  • Caregiver respite and support
  • Meals (hot and nutritious meals to meet the special dietary, cultural and religious needs of the participants)
  • Transportation (available upon request)

Service Highlights

  • Enhanced Easy Access to Services by operating 3 Adult Day Centres at convenient locations and delivering mobile adult day program activities in senior’s buildings in Scarborough and York Region. This year, the program has served over 678 individuals with different ethnicities, complexity of care needs and risk levels with a total of 27,451 attendance.
  • Extraordinary Progress in Transformation of the Adult Day Centre into an INTEGRATE Care Coordination Hub by expanding the wide spectrum of services including individualized assessment and care planning, care coordination and intervention, stimulation programs, recreation therapy, foot care, nutritional counseling, diabetes education and counseling, occupational therapy, physiotherapy, geriatric consultation, primary health care, vision care, homecare, behavioural and caregiver support along with the day program activities provided onsite in the day centre.
  • Revival of the Adult Day Program for Peritoneal Dialysis (PD) Patients with the support from The Scarborough Hospital’s Renal Dialysis Centre. The ADP health care staff received training on how to take care of PD patients, especially in doing the PD process for them.  The initiative is for the purpose of re-opening the series of centre-based programs to PD patients, so that they can access adult day program and other centre-based programs/services offered.
  • Promoted Social Integration and Quality of Life by delivering client and family centred “wrap around bundled care” to ensure the seniors and caregivers can access a wellcoordinated “circle of care”. The services are made available to the seniors and families through the robust collaboration with other services teams, the Carefirst Family Health Team, and the Carefirst Specialist Clinic with the involvement of the family physicians and specialists for medical consultation.
