Chronic Disease Management Centre

Carefirst’s Chronic Disease Management Program (CDMP) is a joint venture between Carefirst Seniors & Community Services Association and Carefirst Family Health Team.  Established in 2011, the CDMP aims to motivate and enable individuals to lead an enhanced quality of life through the provision of community-based chronic disease prevention and management services.


We Envision:

To provide and enable health promotion and chronic disease management services that are integrated, holistic and accessible.

We Serve:

  • Adults who are at risk or living with chronic diseases
  • Caregivers of individuals living with chronic conditions
  • Community health care providers

We Provide:

Programs and services based on community needs, such as:

  • Health Support Group – for example: Renal Support Group, Anxiety & Stress Support Group
  • Health workshops – for example: Healthy Cooking Class, Caregiver Workshop Series, Skincare Workshop, Weight Management Clinic
  • Chronic disease prevention and rehabilitation programs – Community Cardiovascular Prevention and Rehabilitation Program, Chronic Obstructive Pulmonary Disease (COPD) Rehabilitation Program
  • Individual counselling
  • Diabetes Education and Prevention Program

We are a team of dietitians, exercise therapists, family physicians, nurses, social workers, and a network of health care collaborators in the community.

Service Highlights

  • Seamless Access to Services – The Chronic Disease and Management Program (CDMP) continues to serve as a Hub and Spoke platform to support the Scarborough Family Health Organization by providing their patients with access to Dietary, Physiotherapy, and Social Work Intervention services along with other community support services provided by Carefirst.

The Program also provides access to four very unique community-based disease specific rehabilitative and self-management initiatives including Cardio Rehabilitation Program, Chronic Obstructive Pulmonary Rehabilitation Program, Assess and Restore and Pain Management Program.

  • Expanded Scope of Services – Carefirst is proud to have The Geriatric Assessment and Intervention Network (GAIN) team amongst the many services it provides the community. GAIN provides specialized geriatric care to support frail seniors living at home or in retirement residences, who have multiple complex medical and social problems including:
    • Cognitive impairment
    • Responsive behaviours
    • Decreased function
    • Falls or risk of falls
    • Impaired mobility
    • Incontinence and/or
    • Multiple medications

The interdisciplinary team of health care professionals that make up GAIN include a Pharmacist, Social Worker, Nurse Practitioners, Occupational Therapist, and Behavioral Support Nurse collaborate with clients and caregivers to enable them to live as independently as possible in the community with an optimal quality of life.

  • Evidenced-Based Programming – Our CDMP programs are grounded in best-practice standards and align with evidenced-based guidelines. Participants are taught how to prevent or manage chronic diseases using a multidisciplinary approach with the goal to enable them to self-manage their health conditions. The programs have a health education component taught by a team of health care professionals including Dieticians and Registered Nurses, and a supervised exercise component overseen by Physiotherapists and Kinesiologists.
