Carefirst Family Health Team (FHT) continues to embrace the core values for quality improvements, which are embedded in the day to day operation of the FHT.

Access Clinic

The Carefirst Access Clinic in Markham, Richmond Hill, and Scarborough provides same- or next-day episodic primary care for people age 10 and older with non-emergency health concerns. Our Nurse Practitioners, supported by a team of allied health professionals, offer assessment, treatment for non-emergency health concerns, and connections to other necessary services. This clinic is especially helpful if you do not have a family doctor, while you are waiting for enrollment into Carefirst Family Health Team. Anyone aged 10 and over with a valid OHIP card is welcome.

Chronic Disease Management Program

The goal of CDMP is to move from being reactive to being proactive. Through group education sessions and one-on-one support for regular monitoring, we give you the tools and confidence to self-manage your health at home. By staying ahead of these conditions together, we can prevent complications and keep you feeling your best as you age. It is not just about medical treatment; it is about empowering you with the knowledge to stay healthy.

Diabetes Education

Our program provides current, practical, culturally-relevant and customized diabetes education to help patients make decisions which will improve their blood glucose control. The program has multidisciplinary professionals’ back up to answer patients’ concerns.
