Carefirst Family Health Team Receives Major Funding to Expand Services

We are thrilled to announce that Carefirst Family Health Team (FHT) has received $4,644,900 in new provincial funding to support a significant expansion of primary care services across the York Region and Scarborough. This funding will enable the establishment of three new clinics—one in Richmond Hill and two in Markham, along with an expansion of our existing Scarborough site.

This exciting development is part of Ontario’s $2.1 billion Primary Care Action Plan, designed to ensure that every Ontarian has access to a family doctor or primary care team by 2029. Through this plan, Carefirst FHT will play a key role in helping to connect thousands of new patients to high-quality, team-based primary care—an essential step toward building healthier, stronger communities. In Richmond Hill alone, a dedicated investment of $1,890,400 will help connect up to 5,575 residents who currently lack access to a regular primary care provider. Carefirst FHT will prioritize individuals who are currently unattached to care, including those on the Health Care Connect waitlist.

Carefirst FHT will continue to lead in providing a comprehensive and collaborative model of care, integrating family physicians, nurse practitioners, nurses, social workers, dietitians, physiotherapists, and administrative staff. Our services go beyond routine medical care – with strong community partnerships and specialized services such as a Chronic Disease Management Program and a Diabetes Education Program, Carefirst remains committed to patient-centred care at every level to meet the needs of our diverse communities.

This expansion was made possible thanks to the advocacy and support of local MPPs and the Ontario Ministry of Health. We extend our gratitude for their continued commitment to improving access to care in our region.

We are excited to continue growing our services to meet the evolving needs of our communities. Stay tuned for more updates, including details on how patients can sign up for services at the new clinics.
