Locally Driven Population Health Models (LDPHM)
Celebrate Health Fest 2024
On September 14, 2024, Carefirst co-hosted the Celebrate Health 2024: Creating The Right Opportunities event with Eastern York Region Ontario Health Team and Markham YMCA. Hundreds of fairgoers from Southeast Markham and surrounding communities came to learn more about the importance of taking charge of our holistic wellness. Educational health workshops were delivered in different languages on topics such as mental wellness, caregiver resources and cooking demonstrations. Attendees also had access to a mobile health clinic offering cancer screenings, pap smears, and a health screening area for blood pressure, falls prevention and more. The event also featured a community ambassador-led theatre performance addressing myths on mood. Throughout the day, guests could explore service information and resource booths, participate in family fun activities like balloon art, music, art, Henna, and receive free giveaways, and refreshments. A food drive was also held to support Markham Food Bank and 105 Gibson Food Bank.
Carefirst board members, partner representatives, MPP Billy Pang, City Councillor Isa Lee, and School Board Trustee Ron Lynn were in attendance to share their support. Thank you to all the staff, LDPHM program partners, EYRND OHT, Markham YMCA, medical professionals and experts, volunteers, and community members for coming out to participate and making this event a success.
The High Priority Communities Strategy Program is now known as Locally Driven Population Health Models(LDPHM). If you would like any of the festival or workshop information, please contact the Team at 437-772-3415 or [email protected].
